The Tree City USA award is made in recognition of work completed by the city during the calendar year. Please provide information for the year ending. This application is only intended for recertification. This is not a first-year application.

In addition to the information you will provide below, later on you will be asked to attach copies of:

• Public tree care ordinance
• Annual work report outlining the work carried out during the past year
• Breakdown of community forestry expenditures
• Program of activities and news coverage
• Your Arbor Day proclamation

Some states require information in addition to that requested on this application. Check with your state forester.
